DH-Data is produced by the UK Department of Health (DH)
Library and Information Services.
The database reflects the business of the Department of Health
and its Agencies and includes all DH publications including
circulars and press releases. Core subjects include health
services management and administration, with an emphasis on
the British National Health Service, including policy,
planning and finance; design, construction and maintenance of
health service buildings; medical equipment and supplies;
medicines safety; public health, nursing and primary care;
mental health; environmental health; social policy; and social
services for families, people with disabilities and older
people.
Use DH-Data to find references on topics such as:
- the commissioning and delivery of health care
- caring for elderly people in residential homes
- standards for the design and construction of hospital
buildings
- published toxicity studies on specific chemicals

Sources
Articles are indexed from over 300 English-language journals,
together with books, reports, pamphlets, administrative
circulars and other official publications. An important feature
of the database is the inclusion of full bibliographic details,
including sources of supply and URLs (where available) of
Department of Health publications. Approximately one third of
documents have abstracts.

This report explores the causes and frequency of medication
errors, highlights drugs and clinical settings that carry
particular risks, and identifies models of good practice to
reduce risks. The NHS is already rich in examples of good
practice, and report draws on these, together with
experience in North america and elsewhere, to describe a
wide range of measures that will drive down the risk of
medication errors. It contains good practice recommendations
in areas which are known to be error prone. These are
intended to help NHS organisations and professionals examine
current practice to make medication safer for patients.

Primary care organisations (PCOs) in England are required to
run a prescribing incentive scheme. The average payment
received by general practitioners (GPs) under these schemes
is not known. The authors conducted a longitudinal (two year)
questionaire study of all PCOs in London and the south east
of England aiming to explore the relationship between the
financial incentives, the selection of prescribing indicators
and success at remaining within budget. In the second year,
the average reward per GP amounted to 1,220 (range 470-
4,330). Underspent PCOs made larger incentive scheme payments
to their practices as did PCOs that had successfully
overturned a first year overspend into a second year
underspend. The size of rewards was unrelated to the
selection of any particular cost- or quality-based
prescribing indicator. The authors conclude that larger
prescribing incentive scheme payments may have contributed
to prescribing cost control but their effect on prescribing

DH-Data Thesaurus Terms:
The DE paragraph contains descriptors from the DH-Data
Thesaurus. These terms are also available in their own
